Login Information

I tried to login with my Login ID and Password, but I get the error message "Login Failed". What do I do?
The password is case sensitive. Please be sure to enter the password exactly the way it is written in your confirmation email or the way you have entered it after changing the initial computer-generated password.

I'm filling out the registration form, and it's telling me my email address or tax ID is already in use.
There are two possibilities.

The first is that someone at your company has already registered with Kaiser Permanente, or that your company is already in Kaiser Permanente's database. In this case, go to the Vendor Login page and click the "Forgotten Your Password?" link. Enter the email address in the box in the pop-up window and click submit. The password will be sent to the email address.

The other possibility is that you have just registered, and you're trying to submit the registration form again. Please go to the Vendor Login page and click the "Forgotten Your Password?" link. Enter the email address in the box in the pop-up window and click submit. The password will be sent to the email address.

Are There Any Possible Browser Compatibility Issues With This Registration System?
Users may encounter problems when completing their profile information if using the Google Toolbar and IE6 . Fields may appear to be protected and users will be unable to enter required information. This issue can be resolved by temporarily uninstalling the Google Toolbar when completing the profile information.

What does SBA CCR mean?
Small Business Administration Central Contractor Registration (SBA CCR). The Central Contractor Registration (CCR) is the primary vendor database for the U.S. Federal Government. The CCR collects, validates, stores and disseminates data in support of agency acquisition missions.

What is an NAICS Code?
The North American Industry Classification System (NAICS) is used to categorize what type of service/product a vendor provides.

Business Ownership Classification

Supplier Diversity Programs in all industries include several components, such as supplier outreach, supplier registration, and reports. Kaiser Permanente asks a supplier to indicate its business ownership status, as a part of the routine supplier diversity registration process. We then determine if the business meets the criteria for our Program, and track and report information about the businesses with which we purchase and or contract.

The federal government requires us to report the number of suppliers by various classification, including ethnicity (Caucasian, African American, Hispanic American, Native American, Asian American), gender and size of the business (small or large), as well as HUBZone and veteran classification. Kaiser Permanente tracks the information, as part of its own program requirements to monitor progress.

Please note that answering any of the following questions is not a mandate from Kaiser Permanente, nor does it confirm or deny obtaining any future business with Kaiser Permanente. However, to be included in Kaiser Permanente's Supplier Diversity Program, a company is required to declare its ownership classification.

Pick a best match for NAICS. There will be an opportunity in the registration form to further define the service/product your company can provide to Kaiser Permanente.

I clicked the number (NAICS Code), and nothing happened. What's wrong?
If you didn't click on a 6-digit code, you might have just expanded the list for what you did click. If you scroll down, you should see this list.

If you did click a 6-digit code, it should have been entered into the box on the main form.

Why can't I type in the box (NAICS Code(s))?
Click the "Click here to select" link to get a pop-up window. You can click on the links to branch down to the 6-digit number for the NAICS Code.

Where can I locate a DD Form 214 in order to provide proof of being service-disabled?
